That was pretty useful. Thanks for sharing! We also do a similar sort of analysis in our facility. At times,  the Why Why analysis gets cumbersome. So we go for the Fish bone diagram. And there is one good point in noting the costs of the failure. This will serve to highlight the hidden losses the facility is making due to the problem.
