  • 1.  Failure Codes in Maxmo

    Posted 07-11-2025 10:06 AM

All,  our team is working on an effort to evaluate the failure codes currently set up in Maximo for our company.   Maximo used the PCR (Problem, Cause, Remedy) hierarchy method.   Our business most closely follows food and beverage manufacturing practices.   Does anyone have a list they would be willing to share for setting one of these that I could reference.   I'm aware of the ISO 14224 standard but it tends to have many gaps with respect to using the PCR method coupled with Petro-Chemical vs Food and Beverage especially around areas of controls and electrical.   Pump, valves, gearboxes, power distribution and piping seems to be sufficient matches.   Note: even though these were set up in our system many years ago,  they have never been used so I can't rely on historical data based on usage to guide our analysis either.

Problem, Cause and Remedy codes should be equipment or component agnostic so can be applied to any industry. If you need to add that information then another field is required with a drop down list, or lists if you want it to be equipment and component data.

I never allow free text entry or a code to indicate Other or similar non-specific codes to drive picking the nearest match and getting feedback for code additions.

You might consider looking at the failure library that comes with the Reliability Strategies add-on for MAS Manage (if you have upgraded from Maximo 7.6.x). While some of the information contained in the library is arguable (as with any source of information) it does have rather extensive lists for the common equipment. And yes, as Andrew Lane says, be sure your list does not include terms like: Other, Misc, N/A, etc. ... and be sure it is easy for folks to suggest additions to the PCR tree and that their suggestions are added quickly. Thanks so much for the question.
