  • 1.  Which is better FMEA or RCA

    Hello,

    In order to decrease the failure rate, identify unreliable components of the equipment, which is preferred FMEA or RCA. Can someone please throw light on this?

    Start with FMEA/FMECA to determine the correct maintenance strategy.  Begin with the most likely failure modes.  Implement the strategies and if and when you do experience a failure that you didn't capture during the FMEA, then use RCA/RCFA to address that failure mode.

    You should use both as they serve different purposes.  FMEA is a proactive analysis of equipment to understand what failure modes exist, what their impacts are, and then determine how to proactively reduce or detect these potential failures.  RCA is a reactive analysis after a piece of equipment failed to understand why it did and then take remedial steps to prevent it from happening in the future.

     

    If you had to prioritize one over the other, I would suggest FMEA as it should help you prevent failures and therefore reduce the number of times you need to do RCA.

    I like Steven's answer. It's spot on. RCA is like a rifle shot: one condition. FMEA is an analysis of the equipment. It works well developing the PM's and PdM's that are modified for the equipment's specific operating envelop. I like working that with a team of engineers, operators, and maintenance personnel who work with that specific piece of equipment. 

    The issue with FMEA is that if you include every component of a complex piece of equipment, it becomes arduous and time consuming. You can narrow the focus of FMEA by focusing on the main mission of the equipment. What is it that the equipment must always do to perform its function in the production process? Spend  your time on those components that are mission critical. Work them all the way through to identifying what specific actions, at what frequency, should they be performed to avoid the failure mode, i.e., PM's and PdM's.

    BTW, frequency can be establish based on past failure rates. The best frequency is that which has a 98% confidence level based on the history.

    This is not a choice. FMEA is an analysis of what could happen in worst case scenarios, possible damage, mitigations, etc. RCA is a postmortem, analysis reactive to an event and how to avoid a repeat. Either will help decrease failure rate. Correctly performed FMEA will decrease the need of RCA by decreasing unexpected events.
