Broken shaft failure analysis

    Hi All,

    I am looking for someone in the US who can analyze a broken shaft to help us identify the cause of failure. The shaft is off of a roller for an ASRS-style crane system. The vendor/manufacturer is not in the US so we are looking for an outside source for failure analysis.

    Acuren is really good. 
    We used them for Crankshaft and main bearing failure analysis and their results were impressive.
    Although these kinds of inspections can be really pricey.

    I believe Applied Technical Services (ATS) can do the analysis for you.   They did a great "Practical Plant Failure Analysis" course for us back in 2018.  This course covered shaft failure.

    Stress Engineering, Inc. is a company that can provide the needed failure analysis.
    See their website at www.stress.com. They have several US offices. Houston TX and Irvine CA may be closest to you. Their test facility is located in Waller TX outside of Houston.
    Excellent reports for both failure analysis and documentation for Continue to Operate evaluations.

    In my opinion the preeminent expert on this type of failure is Neville W. Sachs, P.E.

    His 2007 text entitled "Practical Plant Failure Analysis: A Guide to Understanding Machinery deterioration and Improving Equipment Reliability" published by CRC Press, Taylor & Francis Group and available on Amazon and more to your problem "Failure Analysis Made Simple: Shafts and Fasteners Paperback" – January 1, 2018 Published by Reliabilityweb .com MROZONE bookstore, also available through Amazon.     Ryan,

    I would agree with both Bud Lockwood (on Stress Engineering) and Jack Nicholas (on Neville Sachs). Neville is the "Godfather" in Failure Analysis of equipment within industrial settings. Many of his experiences (i.e. steam turbines) cannot be acquired any long due to evolution.

    If a more practical failure analysis approach is desired, Mike Barkle, one of our Sr. Reliability Engineer (mikebarkle@infralogix.com), is highly qualified to perform this type of work. He has helped many clients with a more practical initial evaluation to determine any obvious actionable conclusions before seeking formal FA evaluations. He also has experienced with ASRS-style cranes since several of our clients have such systems. Feel free to email him directly to inquire about suitability.

    I would also recommend Neville Sachs.  He has been instrumental in providing training and failure analysis services for the ski lift maintenance industry.
