    Always expect documents to be improved. Year over year, there might be little change but what in the facility is the same as ten years ago? Personnel need to keep up with those changes. Too often, after personnel are trained for a task, they are assumed trained forever.

    Is the calendar full? Make time for training and schedule it for personnel, environment, and equipment safety. Every employee should have time for training each week. Allow them to use the time for company required training and personal growth within the company. Define each position's required knowledge, add at least ten hours for personal growth, divide that by the minimum number of weeks they will work annually. That is the time they need each week.

    How can all that review, and training happen, when is there time? The answer to both is the same. Assign mandatory review and exams according to position annually. If the facility uses an LMS, schedule it there. All employees have an opportunity to suggest revisions from their experience. All employees review all revisions. All documents and employees are up to date.

    Spot-on  "Lockout Larry"!! However, not many organizations do this. But they should!

    It is unfortunately true Terry. It is disappointing how little care is given to document management, creation to update. These documents are the base of maintenance and operations. Inexact and unclear or obsolete documents create unsafe conditions. SME know much about their field but too often they do not know how to write a document.
